US99.5 “FNPP Professional Bullriders Ticket Giveaway”

 

 

a)      The WUSN (the “Station”) “FNPP Professional Bullriders Ticket Giveaway” contest (the “Contest”) is a promotion that will take place on the following dates January 8, 2010 at Murphy’s Bleachers, located at 3655 N Sheffield in Chicago, IL from 9pm to 11pm CT, on January 15^th, 2010 at Rockwelz located at 11265 W 159^th Street in Orland Park, IL from 9pm to 11pm CT, on January 22^nd at TBD location and on January 29^th at TBD location.

 

b)      To participate in the Contest, listeners must stop by Murphy’s Bleachers, located at 3655 N Sheffield in Chicago, IL from 9pm to 11pm CT, on January 15^th, 2010 at Rockwelz located at 11265 W 159^th Street in Orland Park, IL from 9pm to 11pm CT, on January 22^nd at TBD location and on January 29^th at TBD location to obtain and fill out an official entry form, available on location while supplies last. Contestants must provide all information requested, including their first and last name, complete address, including zip code, daytime telephone number and date of birth in order to be eligible for a chance to win. Entry forms must be dropped in the designated drop box or handed to the appropriate representative once completed. Entries must be received before each drawing. Drawings will take place at the end of the station appearance. All entries must be received by 10:59PM CT to be eligible.

 

c)      The Station will select five (5) potential winners in a random drawing from all entries received. Each selected winner will receive two (2) tickets to the Professional Bulldriders on February 5, 2010 at the Sears Centre, located in Hoffman Estates, Illinois, upon confirmation of Contest eligibility. Potential winners will have 3 minutes to claim tickets before an alternate winner be selected. 

d)     Must be 21 years or older and must be present to win.

 

e)      One (1) entry per person, please. No photocopies or mechanically reproduced entries will be accepted.

f)       The Station and the participating sponsors of this Contest (hereinafter collectively referred to as the “Sponsors”) assume no responsibility whatsoever for any lost, misdirected, illegible, stolen, incomplete, mutilated, or late entries. All entries become the exclusive property of the Station and will not be returned.

 

a)      The Contest is open to all Station listeners who reside in the Chicagoland Area and Northwest Indiana, and who are 21 years of age or older at the time of entry. Employees of the 4 listed FNPP locations, the Station, its licensee, corporate parents, subsidiaries and affiliates, its advertising agencies, participating sponsors/promotional partners, and other radio stations in the Chicagoland Area and immediate families of such persons and those living in the households of same are not eligible to participate or win. 

 

b)      Entry into the Contest constitutes your agreement to be bound by these Official Contest Rules.

 

c)      Winners will be required to present valid government issued photo identification and to sign all required releases in order to claim their prize.

 

 

 

 

 

 

a) Decisions of Station management with respect to the Contest are final.

 

b)      Five (5) winners will be selected at each station appearance in the manner set forth in Section 1 of these rules. 

 

c)      Winners must be present to win. Winners will be notified of their prize on location. If a winner is not present when their name is called, an alternate potential winner will be selected in a random drawing of all entries and announced until an eligible winner is found.  If a person is not present when their name is called, their name will not be returned to the entry box. 

 

d)     Odds of winning depend upon the number of eligible entries received.

 

 

a)      Payments of all federal, state, and local taxes are solely the responsibility of the winner. Winners may be required to sign a W-9 IRS Form with the winner’s full Social Security Number or the equivalent for receipt of any prize valued at $600 or more or for any prizes awarded by the Station in a calendar year with an aggregate value of $600 or more. Failure to submit a complete W-9 or equivalent will result in forfeiture of the prize.

 

b)      By participating in the Contest, the winners agree to have their name, city, state, voice, or likeness used in any advertising or broadcasting material relating to this Contest without additional financial or other compensation, and, where legal, to sign a publicity release confirming such consent prior to acceptance of the prize. 

 

c)      Prior to awarding any prize or prize certificate, the Station, in its sole discretion, may require Contest winners and their guests to sign a liability release agreeing to hold the Station, its corporate licensee, its parent, subsidiary and affiliated corporations and the officers, shareholders, directors, employees, agents, and representatives of each of them harmless against any and all claims or liability arising directly or indirectly from the prize or participation in the Contest.

 

d)     The Station reserves the right to make changes in the rules of the Contest which will become effective upon announcement, and to substitute a prize or prizes of equivalent value should a prize become available. If due to circumstances beyond the control of the Station, any competition or prize-related event, including the Josh Turner Concert at the Rialto Square theater on April 22, 2009 is delayed, rescheduled, postponed or cancelled, the Station reserves the right, but not the obligation, to cancel or modify the Contest and shall not be required to award a substitute prize.

 

e)      Failure to comply with the Official Contest Rules may result in a contestant’s disqualification at the sole discretion of the Station.

 

f)       The Station is not responsible for typographical or other errors in the printing, the offering, or the administration of the Contest, or in the announcement of the prizes.
