Official Contest Rules
NO PURCHASE NECESSARY TO ENTER OR WIN. A PURCHASE WILL NOT INCREASE YOUR CHANCE OF WINNING. VOID WHERE PROHIBITED.
1) HOW TO ENTER
a) The US99.5/ Miller High Life Ultimate Handyman contest (the “Contest”) will start on November 7, 2009 at 9:00pm Central Time (CT) and end on November 28, 2009 at 11:00 p.m. CT. The Contest will end when the Grand Prize Winner is selected on Monday, November 30, 2009 at 10:00am CT.
b) To participate in the Contest, stop by one of the US99.5 Rewards Team appearances at the following locations during the dates and times listed: on Saturday, November 7, 2009 at Sara Slammers located at 3601 Channahon Rd., Joliet, IL from 9:00pm to 11:00pm; Saturday, November 14, 2009 at TBD from 9:00pm to 11:00pm; Saturday, November 21, 2009 at Lazer Knights, located at 8950 W. Lawrence Avenue, Norridge, Illinois from 9:00pm to 11:00pm; and on Saturday, November 28, 2009 at Game Time, located at 6405 West 127th Street, Palos Heights, Illinoisfrom 9:00pm to 11:00pm CT. During the appearances, obtain and fill out an official entry form, available on location while supplies last, and drop it in the designated drop box by the conclusion of the event. Entries must include first and last name, complete address, zip code, daytime telephone number and date of birth, in order to be eligible for a chance to win. Winners must be present to win. All entries must be received by the conclusion of each event (at 11:00 p.m. CT at each event).
c) About every thirty (30) minutes during the US99.5 appearance, a member of the US99 promotions department will randomly select one (1) winner from all of the eligible entries received up to that point during the appearance. The person selected will win one (1) $25.00 Home Depot gift card, upon confirmation of contest eligibility, and will qualify for a chance to win the Grand Prize.
d) Oneentry per person. No photocopies or mechanically reproduced entries will be accepted.
e) US99.5 and the participating sponsors of this Contest (hereinafter collectively referred to as the “Sponsors”) assume no responsibility whatsoever for any lost, misdirected, illegible, stolen, incomplete, mutilated, or late entries. All entries become the exclusive property of WUSN and will not be returned.
2) ELIGIBILITY RESTRICTIONS
a) The Contest is open to all US99.5 listeners who reside in the Chicagoland Area and Northwest Indiana, and are 21 years of age or older and homeowners at the time of entry. Employees of Miller Brewing Company, Sara’s Slammers, Lazer Knights, Game Time, WUSN(FM)/US99.5 (the “Station,”) its licensee, its corporate parents, subsidiaries and affiliates, its advertising agencies, participating sponsors/promotional partners, and other radio stations in the Chicagoland Metropolitan Area and immediate families of such persons and those living in the households of same are not eligible to participate or win. The Contest is subject to all applicable federal, state and local laws and regulations.
b) Entry into the Contest constitutes your agreement to be bound by these Official Contest Rules.
c) Winners must show valid government-issued photo identification and sign and return all required releases in order to claim prize.
3) PRIZES
a) Four (4) qualifying prizes will be awarded per event for a total of sixteen (16) qualifying prizes. Each qualifying prize consists of one (1) $25.00 Home Depot gift card Approximate Retail Value (ARV): $25.00. Winners will be subject to the terms and conditions of the gift cards as set forth by their issuer.
b) One (1) Grand Prize will be awarded. The Grand Prize is the “Ultimate Handyman,” prize, including the following: one (1) Dewalt 18V XRP 9 tool combo kit (includes one (1) hammerdrill/drill/driver: DC950, one (1) circular saw: DC390, one (1) reciprocating saw: DC385, one (1) cut-off tool: DC411, one (1) cut-out tool: DC550, one (1) jig saw: DC330, one (1) impact driver: DC825, one (1) impact wrench: DC DW059, one (1) flexible floodlight: DW919, and two (2) 18V XRP batteries, approximate retail value (ARV): $1,298.99.00; one (1) Dewalt heavy-duty worksite radio charger, ARV: $139.00.00; and one (1) Dewalt 3800 PSI, 4.0GPM, Commercial Pressure Washer, ARV: $999.99. (Total Approximate Retail Value (ARV) $2,436.99.) Grand PrizeWinner will be required to provide prize installation. Grand Prize Winner is not permitted to make substitutions to any prize component. There will be no refunds or exchanges for prize components or materials that are not used. If the winner chooses to forfeit the “Ultimate Handyman” prize, the prize in its entirety will be forfeited. Prize provider may substitute equal value materials or prize components at its discretion. The Station is not responsible for any property damage or personal injury in relation to this prize.
c) All prizes or prize certificate may be picked up at the Station’s address below. The winner will forfeit any prize certificate not claimed within forty-five (45) days of winning. Prize sponsor will be responsible for securing date and time of prize pick up from 2 Prudential Plaza, Suite 1000, Chicago, Illinois, 60601. In the event that a prize or prize certificate is mailed to the winner, it will be with the prior written consent of the winner and therefore, winner assumes the risk of its loss. The Station is not responsible for the safe arrival of a prize or prize certificate.
d) There is no substitution, transfer or cash equivalent for prizes, except that Sponsor may, at its sole discretion, substitute prizes of comparable value or cash. The prizes are expressly limited to the item(s) listed above and do not include taxes, gratuities or any other expenses. Other restrictions may apply.
4) SELECTION OF WINNERS
a) Decisions of Station management with respect to the contest are final.
b) Four (4) qualifying prize winners will be selected at each event in the manner set forth in Section 1 of these rules.
c) One Grand Prize Winner will be selected on Monday, November 30, 2009 at 10:00 a.m. CT by a member of the Station’s promotions department in a random drawing of all qualifying prize winners.
d) Prizes will be awarded upon confirmation of Contest eligibility. Qualifying prize winners will be notified of their prize on location upon selection. If a qualifying prize winner is not present or does not claim prize within three (3) notification attempts after the name has been called, another qualifying prize winner will be drawn until an eligible winner is found.
e) Grand Prize Winner will be notified by telephone. If the Grand Prize Winner cannot be reached within three (3) attempts in five (5) business days or cannot accept prize for any reason, he/she will forfeit prize and an alternate Grand Prize Winner will be selected in a random drawing of the remaining qualifying prize winners.
f) Qualifying prize winners need not listen to win but must be present to win and qualify for the Grand Prize. Grand Prize Winner does not need to listen or be present to win).
g) Odds of winning a qualifying prize depend upon the number of entries received at each event. Odds of winning the Grand Prize are 1 in 16.
h) Winners must execute and return any required Affidavit of Eligibility and/or Release of Liability Prize Acceptance Form within forty-five (45) days of winning or prize will be forfeited and an alternate winner will not be chosen.
5) CONDITIONS
a) Payments of all federal, state and local taxes are solely the responsibility of the winners. Winners will be required to complete and submit an IRS Form W-9 with the winner’s full Social Security Number or the equivalent for receipt of any prize valued at $600 or more or for any prizes awarded by the Station in a calendar year with an aggregate value of $600 or more. Failure to submit a complete W-9 or equivalent will result in forfeiture of the prize.
b) By participating in the Contest, winners agree to have their names, voices, or likenesses used in any advertising or broadcasting material relating to this Contest without additional financial or other compensation, and, where legal, to sign a publicity release confirming such consent prior to acceptance of the prize.
c) Prior to awarding any prize or prize certificate, the Station, in its sole discretion, may require contest winners to sign a liability release agreeing to hold the Station, its corporate licensee, its parent, subsidiary and affiliated corporations and the officers, shareholders, directors, employees, agents, and representatives of each of them harmless against any and all claims or liability arising directly or indirectly from the prize or participation in the contest.
d) The Station is not responsible for the quality of the prizes or prize components furnished by Contest sponsor.
e) The Station reserves the right to make changes in the rules of the Contest, including the substitution of a prize of equivalent value, which will become effective upon announcement. If due to circumstances beyond the control of the Station, any competition or prize-related event is delayed, rescheduled, postponed or cancelled, the Station reserves the right, but not the obligation, to cancel or modify the Contest and shall not be required to award a substitute prize.
f) Failure to comply with the Official Contest Rules may result in a contestant’s disqualification solely at the discretion of the Station.
g) The Station is not responsible for typographical or other errors in the printing, the offering, or the administration of the contest, or in the announcement of the prizes.
