1. HOW TO ENTER
a. WUSN’s/FOX’s GLEE Giveaway” website contest (the “Contest”) will begin on Thursday, November 5th, 2009 at approximately 5:00 PM Central Time (“CT”) and end on Monday, November 30th, 2009 at approximately 5:00 PM CT (the “Contest Period”).
b. To participate in the Contest, you must enter via the following method: Beginning on Thursday, November 5th, 2009 at 5:00 PM CT, log onto www.us99.com, click on the “Contests” page link, and click on the “FOX’s GLEE” contest link. Enter your first and last name, complete address, city, state, zip code, home telephone number, date of birth, a valid email address, and correctly answer the weekly GLEE question. One (1) Internet entry per person per week and one (1) Internet entry per email address per week is permitted. The first GLEE question will become available to answer at approximately 5:00PM on November 5th. At 12:00AM on Tuesday, November 17th the first GLEE question will change, thus ending the first week’s entry period. Registration begins for the 2nd GLEE question will begin at approximately 12:01AM on November 17th. At 12:00AM on Tuesday, November 24th the second GLEE question will change, thus ending the second week’s entry period. Registration begins for the 3rd GLEE question at approximately 12:01AM on November 24th. The contest period ends at 11:59PM on Monday, November 30th. Internet entries will be deemed made by the authorized account holder of the email address submitted at the time of entry. The authorized account holder is the natural person who is assigned to the email address by an Internet access provider, online server provider or other organization that is responsible by assigning email address or the domain associated with the submitted email address. WUSN is not responsible for entries not received due to difficulty accessing the Internet, service outage or delays, computer difficulties or other technological glitches. All online entries must be received before the end of each weekly period. For purposes of these Official Rules, “receipt” of any online entry will be solely determined when the Contest administrator’s website server records the entry date. Any automated computer form reply or receipt (such as one confirming delivery of email) does not constitute actual receipt by Administrator of an entry form for purposes of these Official Rules.
c. WUSN is not responsible for entries not received due to difficulty accessing the Internet, service outage or delays, computer difficulties or other technological glitches
2. ELIGIBILITY RESTRICTIONS
a. The Contest is open to all persons who are 18 years of age or older and reside in the Greater Chicago Land Metropolitan Area in Illinois, Indiana and Wisconsin (Cook, Will, DuPage, Kane, Kenosha counties). Employees of WUSN (the “Station”), its licensee, CBS Radio inc., its corporate parents, subsidiaries and affiliates, advertising agencies, participating sponsors/promotional partners, other radio stations in the Greater Chicago Land Metropolitan Area in Illinois, North-west Indiana and Southern Wisconsin and the members of their immediate families and/or households are ineligible to participate or win. Subject to all applicable federal, state, and local laws and regulations.
c. Winner must hold a valid government photo identification and sign and return any required Affidavit of Eligibility and/or Publicity/Liability Release in order to claim prize.
d. Contestants are required to provide truthful information and the Station will reject and delete any entry that it discovers to be false or fraudulent. The Station will disqualify any entry from individuals who do not meet the eligibility requirements, and the Station will also delete any entry knowingly received from persons under the age of 13 in compliance with the Children’s Online Privacy Protection Act.
3. PRIZES
a. Three (3) weekly qualifying prizes will be awarded. The weekly prize is a pair (2) of tickets to see Jake Owen on December 4th at Joe’s Bar located at 940 Weed Street in Chicago. Approximate retail value: $40.00
b. One (1) grand prize will be awarded. The grand prize consists of one (1) $250 Gift Card toward the concert of choice, one (1) 8GB iPod Nano with camcorder and FM radio ($175), one (1) iTunes gift card ($50), and one (1) Home Cube ($60). Grand Prize approximate retail value: $535.00
b. There is no substitution, transfer, or cash equivalent for prizes, except that Station may, at its sole discretion, substitute prizes of comparable value or cash. The prizes are expressly limited to the item(s) listed above and do not include taxes, gratuities or any other expenses. Other restrictions may apply.
c. All prizes or prize certificates will be mailed to the winner after winner signs and returns the required releases. WUSN is not responsible for the safe arrival of a prize or prize certificate.
4. WINNER SELECTION AND NOTIFICATION
a. Decisions of WUSN management with respect to the Contest are final.
c. On Tuesday, November 17th, 2009 at approximately 12:00 PM CT, one (1) qualifying winner will be selected in a random drawing from all eligible entries received by the entry deadline of 12:00AM on November 17th. On Tuesday, November 24th, 2009 at approximately 12:00 PM CT, one (1) qualifying winner will be selected in a random drawing from all eligible entries received by the entry deadline of 12:00AM on November 24th. On Tuesday, December 1st, 2009 at approximately 12:00 PM CT, one (1) qualifying winner will be selected in a random drawing from all eligible entries received by the entry deadline of 11:59AM on November 30th.
d. On Tuesday, December 1st one (1) grand prize winner will be randomly chosen from the pool of three qualifying prize winners.
c. Odds of winning qualifying prize depend on the number of eligible entries received.
Odds of winning grand prize depend 1 in 3.
d. Winner need not be present.
e. If the winner cannot be contacted within five (5) days of the initial notification attempt, or does not execute and return any required Affidavit of Eligibility and/or Release of Liability Prize Acceptance Form within five (5) days of the notification, the prize will be forfeited and an alternate winner will be selected in a random drawing from all remaining eligible entries.
5. CONDITIONS
a. Payments of all federal, state and local taxes are solely the responsibility of the winner. The winner will be required to complete and submit an IRS Form W-9 with the winner’s full Social Security Number or the equivalent for receipt of any prize valued at $600 or more or for any prizes awarded by the Station (or any other CBS Radio station in Station’s market) in a calendar year with an aggregate value of $600 or more. Failure to submit a complete W-9 or equivalent will result in forfeiture of the prize. Such winnings of $600 or more will be reported to the IRS.
b. By participating in the Contest, winner agrees to have his/her name, voice, or likeness used in any advertising or broadcasting material relating to this Contest without additional financial or other compensation, and, where legal, to sign a publicity release confirming such consent prior to acceptance of the prize.
c. Prior to awarding any prize or prize certificate, the Station in its sole discretion may require Contest winner to sign a liability release, agreeing to hold the Station, its corporate licensee, its parent, subsidiary and affiliated corporations, and the officers, shareholders, directors, employees, agents and representatives of each of them harmless against any and all claims or liability arising directly or indirectly from the prize or participation in the Contest.
d. The Station, in its sole discretion, reserves the right to disqualify any person tampering with the entry process, the operation of the Station’s Website or who is otherwise in violation of the rules. It further reserves the right to cancel, terminate or modify the Contest if it is not capable of completion as planned, including infection by computer virus, bugs, tampering, unauthorized intervention or technical failures of any sort.
e. The Station reserves the right to make changes in the rules of the Contest, including the substitution of a prize of equivalent value, which will become effective upon announcement. If due to circumstances beyond the control of the Station, any competition or prize related event is delayed, rescheduled, postponed, cancelled, or has a change of venue, the Station reserves the right, but not the obligation, to cancel or modify the Contest and shall not be required to award a substitute prize.
f. Failure to comply with the Contest rules may result in a Contestant’s disqualification solely at the discretion of the Station.
g. The Station is not responsible for typographical or other errors in the printing, the offering or the administration of the Contest, or in the announcement of a prize.
h. Copies of the written Contest rules and a list of winners (when complete) are available during regular business hours at the main studio of WUSN, Two Prudential Plaza, Suite 1000, Chicago, IL 60601.
