Keith Urban Verizon Wireless Rules November 2009

 

 

 

a)      The US99.5 (the “Station”) “Verizon Wireless Keith Urban Ticket Giveaway” Contest (the “Contest”) will take place at three (3) different appearances by the US 99.5 Rewards team: at Verizon Wireless, located at 668 S. Rand Rd. in Lake Zurich, IL on Tuesday, November 10, 2009 from 5:00 p.m. to 7:00 p.m. CT; at Verizon Wireless, located at 209 S. LaSalle in Chicago, IL on Wednesday, November 11, 2009 from 5:00 p.m. to 7:00 p.m. CT; and at Verizon Wireless, located at 12632 S. Route 59 in Plainfield, IL on Thursday, November 12, 2009 from 5:00 p.m. to 7:00 p.m. CT (the “Contest Period”).

 

b)     To participate in the Contest, listeners must stop by Verizon Wireless, located at 668 S. Rand Rd. in Lake Zurich, IL on Tuesday, November 10, 2009 from 5:00 p.m. to 7:00 p.m. CT; at Verizon Wireless, located at 209 S. LaSalle in Chicago, IL on Wednesday, November 11, 2009 from 5:00 p.m. to 7:00 p.m. CT; and at Verizon Wireless, located at 12632 S. Route 59 in Plainfield, IL on Thursday, November 12, 2009 from 5:00 p.m. to 7:00 p.m. CTCT during the Contest Period to obtain and fill out an official entry form, available on location while supplies last. Entrants must provide all information requested, including first and last name, complete address, including zip code, daytime telephone number and date of birth in order to be eligible for a chance to win. Entry forms must be dropped in the designated drop box or handed to the appropriate Station representative once completed. Once a person has entered, he/she will be entered for all drawings that take place during that respective Appearance. All entry forms must be received by 6:55 p.m. at each Appearance.

 

c)      One (1) entry per person, per station appearance. No photocopies or mechanically reproduced entries will be accepted.

 

d)     The Station and the participating sponsors of this Contest (hereinafter collectively referred to as the “Sponsors”) assume no responsibility whatsoever for any lost, misdirected, illegible, stolen, incomplete, mutilated, or late entries. All entries become the exclusive property of the Station and will not be returned.

 

 

a)      The Contest is open to all Station listeners who reside in the Chicagoland Area and Northwest Indiana, and who are 21 years of age or older at the time of entry. Employees of Verizon Wireless, the Station, its licensee, and each of their parent, subsidiary and affiliated entities, their advertising agencies, participating sponsors/promotional partners, and other radio stations in the Chicagoland Area and immediate families of such persons and those living in the households of same are not eligible to participate or win. 

 

b)      Entry into the Contest constitutes your agreement to be bound by these Official Contest Rules.

 

 

 

a)      At each of the Verizon Wireless Appearances, up to twenty-five (25) Prizes will be awarded. Twenty-three (23) winners will receive two (2) passes each to see Keith Urban on Saturday, November 14, 2009, and two (2) winners will each receive two (2) passes each to see Keith Urban on Saturday, November 14, 2009 AND two (2) Keith Urban meet and greet passes at Joe’s Bar, located at 940 W. Weed St., Chicago, IL.  Tickets and/or meet and greet passes will be awarded on-location during each station appearance, once a potential winner’s eligibility has been confirmed. Winners are responsible for all expenses, including but not limited to travel to the event Approximate Retail Value (ARV): $100.00 ($50.00 per concert ticket, no value on meet and greet passes).

 

b)      Winners will each receive a letter from Verizon Wireless with performance details and instructions of how to pick up their tickets on the day of the show. Winners will need to bring this letter and their ID to the venue. ID must match name on list. Contest prizes are not transferable. Prizes may not be substituted or redeemed for cash except that Sponsor may, at its sole discretion, substitute prizes of comparable value or cash. The prizes are expressly limited to the item(s) listed above and do not include taxes, gratuities or any other expenses. Other restrictions may apply.

 

 

a)      Decisions of Station management with respect to the Contest are final.

 

b)      About every fifteen (15) minutes during each Appearance, the US99.5 Rewards Team will select up to three (3) prize winners from all of the eligible entries received up to that point during that Station Appearance. Winners will receive two (2) passes each to see Keith Urban on Saturday, November 14, 2009 at Joe’s Bar located at 940 W. Weed St. in Chicago, IL.  

 

The final drawing will consist of four (4) prize winners. The first two (2) prize winners selected at each Appearance will receive two (2) passes each to see Keith Urban on Saturday, November 14, 2009 at Joe’s Bar located at 940 W. Weed St. in Chicago, IL. The final two (2) prize winners selected at each Appearance will receive two (2) passes each to see Keith Urban on Saturday, November 14, 2009 at Joe’s Bar, located at 940 W. Weed St., Chicago, IL AND two (2) Keith Urban meet and greet passes. Prizes will be awarded upon confirmation of Contest eligibility.

 

 

c)      Winners must be present to win. If winner is not present when his/her name is called or a selected entrant is found to be ineligible, the winner will forfeit the prize and an alternate name will be selected in a random drawing until an eligible winner is found. If a person is not present when their name is called, their name will not be returned to the entry box. 

 

d)     Odds of winning depend upon the number of eligible entries received at the respective Station Appearance.

 

e)      Winners must execute and return and required Affidavit of Eligibility and/or Release of Liability Prize Acceptance Form at the time the prize is awarded or prize will be forfeited and an alternate winner will be selected from all the eligible entries received up to that point.

 

 

a)      Entry into the Contest and acceptance of a prize constitutes permission, unless prohibited by law, to use the winner’s name and guest’s name, voice, or likeness in any advertising or broadcasting material relating to this Contest without additional financial or other compensation.

 

b)      Payments of all federal, state and local taxes are solely the responsibility of the winners. Winners will be required to complete and submit an IRS Form W-9 with the winner’s full Social Security Number or the equivalent for receipt of any prize valued at $600 or more or for any prizes awarded by the Station in a calendar year with an aggregate value of $600 or more. Failure to submit a complete W-9 or equivalent will result in forfeiture of the prize.

 

c)      By entering into the Contest and accepting a prize, the winners (and their guest, if any) agree that the Sponsors, their parents, partners, subsidiaries, and affiliated companies, their respective agents, employees, directors, officers, shareholders and legal advisors and any other companies participating in the design, administration, promotion, advertisement or fulfillment of the Contest (hereinafter collectively referred to as the “Contest Entities”), shall not be liable for personal injuries, death, damages, expenses or costs or losses of any kind resulting from participation or inability to participate in the Contest or acceptance of or use or inability to use the prize(s) or parts thereof including, without limitation, claims, suits, injuries, losses and damages related to personal injuries, death, damage to or destruction of property, rights of publicity or privacy, defamation or portrayal in a false light (whether intentional or unintentional), whether under a theory of contract, tort (including negligence), warranty or other theory. The winners release the Contest Entities from any and all liability and responsibility with respect to any prize (including any property loss, damage, personal injury or death).

 

d)     Prior to awarding any prize or prize certificate, the Station, in its sole discretion, may require Contest winners (and their companion(s), if any) to sign a liability release agreeing to hold the Contest Entities, the Station, its corporate licensee, its parent, subsidiary and affiliated corporations and the officers, shareholders, directors, employees, agents, and representatives of each of them harmless against any and all claims or liability arising directly or indirectly from the prize or participation in the Contest.

 

e)      The Station reserves the right to make changes in the rules of the Contest which will become effective upon announcement, and to substitute a prize or prizes of equivalent value should a prize become available. If due to circumstances beyond the control of the Station, any competition or prize-related event, including the Keith Urban acoustic performance at Joe’s Bar, located at 940 W. Weed St., Chicago, IL on Saturday, November 14, 2009 is delayed, rescheduled, postponed or cancelled, the Station reserves the right, but not the obligation, to cancel or modify the Contest and shall not be required to award a substitute prize.

 

f)       Failure to comply with the Official Contest Rules may result in a contestant’s disqualification at the sole discretion of the Station.

 

g)      The Station is not responsible for typographical or other errors in the printing, the offering, or the administration of the Contest, or in the announcement of the prizes.
