Official Contest Rules
NO PURCHASE NECESSARY TO ENTER OR WIN. A PURCHASE WILL NOT INCREASE YOUR CHANCE OF WINNING. VOID WHERE PROHIBITED.
1) HOW TO ENTER
a) The US99.5/ Pep Boys Free Oil Change Giveaway contest (the “Contest”) will start on November 8, 2009 at 12:00p.m. Central Time (CT) and end on November 8, 2009 at 2:00 p.m. CT.
b) To participate in the Contest, stop by the US99.5 Rewards Team appearance at Pep Boys, located at 1020 Summit Street, Elgin, IL from 12:00 P.M. CT to 2:00 P.M. CT (the “Contest Period”). During the appearance, obtain and fill out an official entry form, available on location while supplies last, and drop it in the designated drop box by the conclusion of the event. Entries must include first and last name, complete address, zip code, daytime telephone number and date of birth, in order to be eligible for a chance to win. Winners must be present to win. All entries must be received by 2:00 P.M. CT.
c) About every thirty (30) minutes during the US99.5 appearance from 12:00 p.m. CT to 2:00 p.m. CT, a member of the US99 promotions department will select two (2) winners from all of the eligible entries received up to that point during the appearance. The person selected will win one (1) voucher for a free oil change redeemable at the Pep Boys Elgin Service Center located at 1020 Summit Street, Elgin, IL anytime before December 31, 2009, upon confirmation of contest eligibility.
d) Oneentry per person. No photocopies or mechanically reproduced entries will be accepted.
e) US99.5, Pep Boys, and the participating sponsors of this Contest (hereinafter collectively referred to as the “Sponsors”) assume no responsibility whatsoever for any lost, misdirected, illegible, stolen, incomplete, mutilated, or late entries. All entries become the exclusive property of WUSN and will not be returned.
2) ELIGIBILITY RESTRICTIONS
a) The contest is open to all US99.5 listeners who reside in the Chicagoland Area and Northwest Indiana, and are 18 years of age or older at the time of entry. Employees of Pep Boys, WUSN (FM)/US99.5 (the “Station”), its licensee, its corporate parents, subsidiaries and affiliates, its advertising agencies, participating sponsors/promotional partners, and other radio stations in the Chicagoland Metropolitan Area and immediate families of such persons are not eligible to participate or win. Subject to all applicable federal, state and local laws and regulations.
b) Entry into the Contest constitutes your agreement to be bound by these Official Contest Rules.
c) Winner must show valid government-issued photo identification and sign and return all required releases in order to claim prize.
3) PRIZES
a) Eight (8) prize winners will each receive one (1) voucher for a free oil change redeemable at the Pep Boys Elgin Service Center located at 1020 Summit Street, Elgin, IL anytime before December 31, 2009. Approximate Retail Value (ARV): $24.99
b) Prizes will be awarded on-site, upon confirmation of eligibility.
c) There is no substitution, transfer or cash equivalent for prizes, except that Station may, at its sole discretion, substitute prizes of comparable value or cash. The prizes are expressly limited to the item(s) listed above and do not include taxes, gratuities or any other expenses. Other restrictions may apply.
4) SELECTION OF WINNERS
a) Decisions of Station management with respect to the contest are final.
b) Eight (8) prize winners will be selected on November 8, 2009. Two (2) winners will be selected at 12:30 p.m. CT, 1:00 p.m. CT, 1:30 p.m. CT, and 2:00 p.m. CT by a member of the US99.5 promotions department in a random drawing of all the eligible entries received up to that point at the appearance.
c) Prizes will be awarded on location upon selection and confirmation of Contest eligibility. If a prize winner is not present or does not claim prize within three (3) notification attempts after the name has been called, another qualifying prize winner will be drawn until an eligible winner is found.
d) Prize winners need not listen to win but must be present to win.
e) Odds of winning a prize depend on the number of eligible entries received at the event.
f) Winners must execute and return any required Affidavit of Eligibility and/or Release of Liability Prize Acceptance Form within fifteen (15) days of winning or prize will be forfeited and an alternate winner will not be chosen.
5) CONDITIONS
a) Payments of all federal, state and local taxes are solely the responsibility of the winners. Winners will be required to complete and submit an IRS Form W-9 with the winner’s full Social Security Number or the equivalent for receipt of any prize valued at $600 or more or for any prizes awarded by the Station in a calendar year with an aggregate value of $600 or more. Failure to submit a complete W-9 or equivalent will result in forfeiture of the prize.
b) By participating in the Contest, winners agree to have their names, voices, or likenesses used in any advertising or broadcasting material relating to this Contest without additional financial or other compensation, and, where legal, to sign a publicity release confirming such consent prior to acceptance of the prize.
c) Prior to awarding any prize or prize certificate, the Station, in its sole discretion, may require contest winners to sign a liability release agreeing to hold the Station, its corporate licensee, its parent, subsidiary and affiliated corporations and the officers, shareholders, directors, employees, agents, and representatives of each of them harmless against any and all claims or liability arising directly or indirectly from the prize or participation in the contest.
d) The Station is not responsible for the quality of the prizes or prize components furnished by Contest sponsor.
e) The Station reserves the right to make changes in the rules of the Contest, which will become effective upon announcement. If due to circumstances beyond the control of the Station, any competition or prize-related event is delayed, rescheduled, postponed or cancelled, the Station reserves the right, but not the obligation, to cancel or modify the Contest and shall not be required to award a substitute prize.
f) Failure to comply with the Official Contest Rules may result in a contestant’s disqualification solely at the discretion of the Station.
g) The Station is not responsible for typographical or other errors in the printing, the offering, or the administration of the contest, or in the announcement of the prizes.
