Official Contest Rules
NO PURCHASE NECESSARY TO ENTER OR WIN. A PURCHASE WILL NOT INCREASE YOUR CHANCE OF WINNING. VOID WHERE PROHIBITED.
1) HOW TO ENTER
a) The WUSN Bud Light 10/29Dierks Bentley Ticket Tour contest (the “Contest”) will start on October 29, 2009 at 8:00pm Central Time (CT) and end on October 29, 2009 at 10:55 p.m. CT.
b) To participate in the Contest, stop by one of the WUSN US99.5 Rewards Team appearances at the following locations during the dates and times listed:
i. Thursday, October 29, 2009 at Sequoit Creek Lodge, located at 899 Main Street, Antioch, IL from 8:00pm to 8:30pm,
ii. Thursday, October 29, 2009 at Edison The Limerick, located at 912 Taft Avenue, Antioch, Illinois from 8:45pm to 9:15pm,
iii. Thursday, October 29, 2009 at Anastasia’s, located at 438 Lake Street, Antioch, Illinois from 9:30pm to 10:00pm,
iv. Thursday, October 29, 2009 at Double Play, located at 950 Hillside Avenue, Antioch, Illinois from 10:15pm to 10:55pm CT (The “Contest Period”).
Obtain an “Official Entry Form” available on location, while supplies last, and drop it in the designated drop box by the conclusion of each event. Entry forms must be legible and must include first and last name, complete address, zip code, daytime telephone number and date of birth, in order to be eligible for a chance to win. Winners must be present to win. All entries must be received by the conclusion of each event. Ten (10) winners will be selected from all eligible entries submitted at the conclusion of each station appearance listed above.
Official Stamp Cards are appearance “passports”. Official Stamp Cards are also available, while supplies last, during The Sequoit Creek Lodge appearance only. Contestants must obtain an official stamp on their Cards during each of the four (4) Station appearances listed above by presenting the Stamp Card to an official US99.5 representative a when complete with 4 stamps, one from each location at the Double Play appearance and will receive a pair of tickets to the Dierks Bentley Concert on Saturday, November 7, 2009 at the Libertyville Sports Complex, located at 1950 North US Highway45 in Libertyville, Illinois (Doors at 6:00 p.m. CT, show at 7:00 p.m. CT.)
Stamp cards and Entry forms must be legible and must include first and last name, complete address, zip code, daytime telephone number and date of birth, in order to be eligible for a chance to win. Winners must be present to win. All entries must be received by the conclusion of each event.
At the conclusion of each WUSN (the “Station”) appearance, a member of the Station promotions department will select ten (10) winners from all of the eligible entries received during the appearance. Each winner selected will win one (1) four-pack of tickets to the Dierks Bentley Concert on Saturday, November 7, 2009 at the Libertyville Sports Complex, located at 1950 North US Highway45 in Libertyville, Illinois (Doors at 6:00 p.m. CT, show at 7:00 p.m. CT, upon confirmation of Contest eligibility. At the final event, the Double Play appearance, up to fifty (50) pair of tickets will be distributed to the the eligible Stamp Cards that are submitted. Contestants are eligible to win a four-pack of tickets at each of the 4 locations as well as the Official Stamp Card prize if eligible. Approximate retail value is $10.00 per concert ticket.
c) One entry per person, please. No photocopies or mechanically reproduced entries will be accepted.
d) The Station and the participating sponsors of this Contest (hereinafter collectively referred to as the “Sponsors”) assume no responsibility whatsoever for any lost, misdirected, illegible, stolen, incomplete, mutilated, or late entries. All entries become the exclusive property of WUSN and will not be returned.
2) ELIGIBILITY RESTRICTIONS
a) The Contest is open to all Station listeners who are 21 years of age and older at time of entry, and who reside in the Chicagoland Area and Northwest Indiana. Employees of Anheuser-Busch, Sequoit Creek Lodge, The Limerick, Anastasia’s, Double Play, the Station, its licensee, parents, subsidiaries, other radio stations in the Chicago Metropolitan area and the members of their immediate families are ineligible to participate or win. Subject to all applicable federal, state, and local laws and regulations.
b) All Participants must hold a valid government issued-ID and sign and return any required Affidavits of Eligibility and/or Publicity/Liability Release upon Entry.
c) Entry into the Contest constitutes your agreement to be bound by these Official Contest Rules.
3) PRIZES
Ten (10) prizes will be awarded at each of the four station appearances listed above (40 total). Each prize is one (1) four-pack of tickets to the Dierks Bentley Concert on Saturday, November 7, 2009 at the Libertyville Sports Complex, located at 1950 North US Highway45 in Libertyville, Illinois (Doors at 6:00 p.m. CT, show at 7:00 p.m. CT. Approximate Retail Value (ARV) $40.00 .
In addition, up to fifty (50) prizes will be awarded from all of the eligible Stamp Cards. The prize two tickets to the Dierks Bentley Concert on Saturday, November 7, 2009 at the Libertyville Sports Complex, located at 1950 North US Highway45 in Libertyville, Illinois (Doors at 6:00 p.m. CT, show at 7:00 p.m. CT.) Approximate Retail Value (ARV) of prize is $20.00 ($10.00 per concert ticket.)
a) All prizes will be awarded on-site at each of the four station appearances listed above.
b) There is no substitution or transfer of prizes, except that the Station may, at its sole discretion, substitute prizes of comparable value. The prizes are expressly limited to the cash prize listed above and do not include taxes, gratuities, or any other expenses. Other restrictions may apply.
4) SELECTION OF WINNERS
a) Decisions of Station management with respect to the Contest are final.
b) Up to Forty (40) prize winners will be selected in the manner set forth in Section 1 of these rules.
Each winner will be one (1) four-pack of tickets to the Dierks Bentley Concert on Saturday, November 7, 2009 at the Libertyville Sports Complex, located at 1950 North US Highway45 in Libertyville, Illinois (Doors at 6:00 p.m. CT, show at 7:00 p.m. CT, upon confirmation of eligibility. Winner(s) must be present to win.
Up to fifty (50) prize winners will be selected on Thursday, October 29, 2009 at from all qualifying Stamp Card entries submitted to a member of the Station promotions department. Each winner will receive one (1) pair of tickets to the Dierks Bentley Concert on Saturday, November 7, 2009 at the Libertyville Sports Complex, located at 1950 North US Highway45 in Libertyville, Illinois (Doors at 6:00 p.m. CT, show at 7:00 p.m. CT.)
c) If any winner is not present when his or her name is called, an alternate winner may be selected in a random drawing from the remaining entries received. If a person is not present when their name is called, their name will not be returned to the entry box.
d) Prizes will be awarded upon confirmation of Contest eligibility. Winners will be notified of their prize on location upon selection. If a prize winner is not present or does not claim prize within one (1) minutes after the name has been called, another qualifying prize winner will be drawn until an eligible winner is found.
e) Prize winner will be notified on-site.
f) Odds of winning depend upon the number of eligible entries received at each event. Odds of winning the Official Stamp Card prize depend upon the number of eligible Stamp Card entries received at the final event.
g) Winners must execute and return any required Affidavit of Eligibility and/or Release of Liability Prize Acceptance Form by 5:00 pm CT on November 5, 2009 or prize will be forfeited and an alternate winner will not be chosen.
5) CONDITIONS
a) Entry into the Contest and acceptance of a prize constitutes permission, unless prohibited by law, to use the winner’s name and guest’s name, voice, or likeness in any advertising or broadcasting material relating to this Contest without additional financial or other compensation.
b) Payments of all federal, state, and local taxes are solely the responsibility of the winner. Winners may be required to sign a W-9 IRS Form or the equivalent.
c) By entering into the Contest and accepting a prize, the winners (and their guest, if any) agree that the Sponsors, their parents, partners, subsidiaries, and affiliated companies, their respective agents, employees, directors, officers, shareholders and legal advisors and any other companies participating in the design, administration, promotion, advertisement or fulfillment of the Contest (hereinafter collectively referred to as the “Contest Entities”), shall not be liable for personal injuries, death, damages, expenses or costs or losses of any kind resulting from participation or inability to participate in the Contest or acceptance of or use or inability to use the prize(s) or parts thereof including, without limitation, claims, suits, injuries, losses and damages related to personal injuries, death, damage to or destruction of property, rights of publicity or privacy, defamation or portrayal in a false light (whether intentional or unintentional), whether under a theory of contract, tort (including negligence), warranty or other theory. The winners release the Contest Entities from any and all liability and responsibility with respect to any prize (including any property loss, damage, personal injury or death).
d) Prior to awarding any prize or prize certificate, the Station, in its sole discretion, may require Contest winners (and their companion(s), if any) to sign a liability release agreeing to hold the Contest Entities, WUSN, its corporate licensee, its parent, subsidiary and affiliated corporations and the officers, shareholders, directors, employees, agents, and representatives of each of them harmless against any and all claims or liability arising directly or indirectly from the prize or participation in the Contest.
e) The Station reserves the right to make changes in the rules of the Contest which will become effective upon announcement, and to substitute a prize or prizes of equivalent value should a prize become available. If due to circumstances beyond the control of the Station, any competition or prize-related event, including the Bud Light snow daze event, is delayed, rescheduled, postponed or cancelled, the Station reserves the right, but not the obligation, to cancel or modify the Contest and shall not be required to award a substitute prize.
f) Failure to comply with the Official Contest Rules may result in a contestant’s disqualification at the sole discretion of the Station.
g) The Station is not responsible for typographical or other errors in the printing, the offering, or the administration of the Contest, or in the announcement of the prizes.
